Terms & Conditions:
Cancellations notified
by September 1, 2009 will be refunded in full minus a 10% card
processing fee if a credit card was used for registration (no
processing fee if paid by company checks). Substitutions are
permitted without penalty any time, subject to course prerequisites.
(QFD Black Belt® and Update Courses have prerequisites; please refer
to the respective brochure by clicking the course name).
Registration fees include a copy of the symposium transactions,
course/workshop materials plus Modern QFD templates for QFD course
attendees, continental breakfast, lunch, and coffee breaks. (No
lunch for the half-day QFD Green Belt® Update Course.)
Registration will be
confirmed upon receipt of full payment. Class sizes are limited, and
registration is accepted on a first-come-first-served basis.
Stimulus Offers are good until the special offer seats sell out.
US Federal Government employees with a valid ID may apply a 10%
discount on regular prices but not on Stimulus Offers.
Full-time
undergraduates may apply for
Symposium Scholarship, subject to seat availability.
